Leaves of Absence
We understand you may need to take time off to care for yourself or a loved one. Allegion offers a variety of leaves of absence to help you do just that. Some approved leaves are paid. Others may be unpaid. You may not be eligible for all types of leave, and rules vary depending on the type of leave you need and where you live. We recommend you familiarize yourself with the types of leave, then find out what you need to do to request a leave of absence.

Allegion understands that circumstances may require an employee to be away from work for an extended period of time. An employee may request the following leaves of absence.
We offer various types of paid and unpaid leave, including:
- Family and Medical Leave (FML)
- Parental Leave
- Military Leave
- Personal Leave
- Short-Term Disability
- Long-Term Disability
An employee on a leave of absence will receive benefits and compensation in compliance with Allegion plans/policies and applicable federal and state laws.
An employee on a leave of absence may not perform work for any other employer during that leave except when the leave is for military or public service.
Employees on leave and under a provider’s care MUST submit a return to work note from their provider in order to start working. The Allegion Return from Medical Leave of Absence Form or a note containing the same information and the medical provider’s signature can be submitted.
Benefits deductions for employees who are on leave and not being paid will be paid upon their return to work at no more than double the regular deduction from their pay.
Requesting a Leave of Absence
Please take the following steps if you need to request a leave of absence.
Step 1:
Contact Aflac at 844-426-1325, Monday-Friday, 8am-8pm ET, or file a claim online at mygrouplifedisability.aflac.com/personal, providing the following information:
- Name, address, employee ID and supervisor’s name
- Name of your department and last full day of active work
- Nature of your claim or leave request
- Your treating physician’s name, address, phone and fax number
Step 2:
- You should notify Aflac, your HR Business Partner (HRBP) and/or supervisor at least 30 days in advance of your anticipated leave of absence, unless your need for leave is unforeseen.
- Unforeseen leaves must be reported the day you become aware of your need or the next business day.
- You do not need to provide details surrounding your absence to your manager or HRBP, but you do need to disclose the start date of your leave, your estimated return to work date, if your time away is for your own health condition, to care for a family member, for military duty, parental leave or other personal reason.
- If your estimated return to work date is unknown, you should arrange and agree with your supervisor and/or HRBP on a schedule for regular communication surrounding your intent to return to work.
- Leaves that are not reported to Aflac timely may be delayed or denied.
What Happens to Your Benefits When on a Leave
Your medical, dental, vision, long-term disability, life insurance, accidental death & dismemberment and flexible spending account benefits will continue while on a leave of absence, as long as you continue to make the required contributions.
- For paid leave, your contributions continue to be deducted from your paycheck.
- For unpaid leave, your benefit payments will go into arrears and will be double deducted from your paychecks when you return until the balance is paid in full.
Returning From a Leave of Absence
To return to work, you must have your healthcare provider complete and return Allegion’s Return from Medical Leave of Absence Form directly to the Allegion Benefits team. You can send it directly by email or by secure fax to 1-317-810-3986.
You will be responsible for completing Section 1 of the form, while your healthcare provider must complete Section 2. All documentation must be submitted at least 2 days before returning to work.
Maternity Leave
If you’re a new mom, you also use your short-term disability benefit to take time off to welcome your new child.
