Dependent Eligibility Verification
To remain compliant with our health care plan rules, and to help manage health care costs, Allegion conducts a dependent eligibility verification (DEV) program.
Allegion retains a third-party administrator, Verifi1, to manage our dependent eligibility verification program. During a dependent eligibility verification, you will be required to submit documentation to verify that any dependents you have enrolled in health care coverage through Allegion meet the rules defined by our plan.
From time to time, you may receive updates via email containing information and updates regarding where you stand in the verification process.
Eligible Dependents
You can choose to cover your dependents under Allegion benefits. Your eligible dependents include your:
- Legal spouse
- Child(ren) — natural child, children placed with employee for adoption, legally adopted children and stepchildren up to age 26
- Disabled child of any age, as long as he or she is incapable of self-support due to mental or physical disability, dependent on you for maintenance and financial support, covered by the Allegion Medical Plan at the time deemed disabled and declared disabled prior to reaching age 26
- Child(ren) for whom you are the court-appointed guardian
Please note: Domestic partners are not eligible for coverage under Allegion’s benefit plans.
What You Need To Do
During dependent eligibility verification, you will need to:
- Go to Allegion.verifi1.com and select the “register now” button. Answer the questions to register as a new user.
- Log in to Allegion.verifi1.com using your User-ID, your email address and password you created when you registered. If you forgot your User-ID, you can email allegion.usbenefits@allegion.com.
- Verify the data for each dependent and select the documents you will submit for verification.
- Upload or drag and drop the files into the system (documents must be uploaded for each dependent individually).
- Within three business days, you will receive an email regarding the review of the documents submitted and the status of your dependents.
- If your dependent(s) does not meet the eligibility rules, please log in to Allegion.verifi1.com and voluntarily request that the dependent(s) be removed or check the “remove dependent” box next to the dependent(s) you would like to remove, then scan and upload this document.
If for whatever reason you are unable to log in to Allegion.verifi1.com, and cannot upload your documents to the website, you can fax your documents to 800-209-9201 or email Allegion.USBenefits@allegion.com.
It is your responsibility to ensure that documents are faxed properly and to the correct number.
Eligible Verification Documents
Here are a few examples of documentation you may use to verify the eligibility of your dependent:
- Marriage Certificate
- Federal or State tax return
- A government-issued birth certificate (including parents’ names) for newly added children
- Adoption or guardianship order with court’s seal
